General Information
Please bring your insurance information, including insurance cards, when you come to any location for an appointment. Many medical insurance companies and HMOs require pre-authorization for medical services. Please consult your insurance company or primary care physician to obtain a pre-authorization for services, if required, prior to the date of your appointment.
We accept checks, cash, care credit and major credit cards for payment of services. Co-payments, deductibles and payment of non-covered services are due at the time of service.
